2025-26 Dr. Betty Shabazz Delta Academy Application

The Dr. Betty Shabazz Delta Academy is a national initiative of Delta Sigma Theta Sorority, Inc. that serves girls age 11-14. Delta Academy provides educational and enrichment activities to girls at a crucial time in their development.

This application packet includes:

• Emergency and Medical Information Sheet
• Scholar Information Sheet
• Scholar Code of Conduct
• Parent Code of Conduct
• Scholar & Parent Commitment Agreement
• Photography and Videotape Release Form
• Release of Liability and Attendance Agreement

All information must be completed in its entirety and submitted on the day of orientation

• Late applications will not be considered
• Applicants must be at least age 11 at the time of orientation to participate
