2024-25 Information and Application Packet
This Information and Application Packet includes:
Documents to submit:
• Emergency and Medical Information Sheets
• Signed EMBODI Information Sheet and Commitment Agreement
• Signed EMBODI Code of Conduct
• Signed Photography and Videotape Release Form
• Signed Release of Liability and Attendance Agreement
• Signed Permission Slip for sessions focused on Dating, Relationships, Hygiene, Race & Other Critical Conversations
• Signed Parent Conduct Contract
Documents to retain (will be included in your receipt confirmation email):
• EMBODI Codes of Conduct
• EMBODI Attendance Policy
All information must be completed in its entirety and submitted on or before Saturday, June 1.
Parent/guardian, along with potential program participant, must attend the mandatory Parent & Participant Orientation in September.
• Completed applications must be submitted by the June 1 deadline date.
• Late applications will not be accepted.
• Applicants must be between the ages of 11 and 17.
• Space is limited! An applicant interview may be required for program consideration.
Applicants must live in the Southfield Alumnae Chapter service area:
Southfield, Berkley, Birmingham, Clawson, Farmington, Farmington Hills, Ferndale, Hazel Park, Huntington Woods, Madison Heights, Northville (Oakland County
only
), Novi, Oak Park, Pleasant Ridge, Royal Oak, South Lyon, Milford, Wixom, Royal Oak Township, Lathrup Village, Beverly Hills, Bingham Farms and Franklin.
